根据ROC曲线确定的界值,galectin-3、CEA及CA199诊断胰腺癌的阳性率分别为33.3%、50.0%及63.3%。3项指标中galectin-3虽然敏感性较低,但其有较高的特异性,CEA及CA199对胰腺癌的诊断敏感性相差不多,但CA199诊断特异性要略优于CEA。3项指标都存在诊断敏感性或诊断特异性不太高的缺陷,因此单独检测galectin-3、CEA及CA199诊断胰腺癌均有局限性。联合检测能够在一定程度上弥补单一指标检测的不足,三者对胰腺癌的诊断存在互补价值,二项联合检测组合以galectin-3加CA199最优,既可提高诊断敏感性,又不会明显降低特异性。3项联合检测虽会使诊断特异性有所下降,可明显提高诊断敏感性,对胰腺癌的筛查具有重要临床价值。Galectin-3作为对传统胰腺癌标志物的补充对胰腺癌早期诊断及鉴别诊断具有十分重要的意义,galectin-3有可能成为胰腺癌的重要血清标志物之一。
因本实验中galectin-3的检测为双抗体夹心生物素-亲和素ELISA法,其检测敏感性不太高,不能检测出微量的galectin-3,可能是导致胰腺癌阳性率不太高的原因之一。今后我们将研究开发更敏感的检测方法,进一步扩大病例数,深入研究galectin-3作为胰腺癌标志物的临床意义。
